BATH, NY – Every three years, local health departments and their hospital partners are charged by the New York State Department of Health to conduct a Community Health Assessment (CHA) that results in identified goals outlined in the Community Health Improvement Plan (CHIP). The 2022 – 2024 CHIP for Steuben County is currently under development and can be aided by input from community members.
The Steuben County Public Health Department, along with other area health departments, has partnered with Common Ground Health out of Rochester to launch the My Health Story 2022 survey, available now at MyHealthStory2022.org<https://survey.commongroundhealth.org/pre> and MiHistoriadeSalud2022.org<https://survey.commongroundhealth.org/prs>.
The survey is open to residents, age 18 and older, from Steuben, Chemung, Schuyler, Yates, Livingston, Genesee, Seneca, Wayne, Wyoming, Orleans, Ontario and Monroe counties. Participants can receive a $5 gift card to Amazon or Walmart after completing the survey.
“The voices of the community are very important when considering what the greatest health needs are in Steuben,” said Darlene Smith, Steuben Public Health Director. “Life has changed greatly since our last assessment in 2019, so it is critical to gather input from our residents. This survey is a great tool to reach many, and we hope they will take the opportunity to tell us about their experiences.”
The last My Health Story survey was conducted in 2018 and nearly 7,000 residents across various counties participated. Key findings included:
● Housing impacts health in many ways. People who are stressed about housing payments are more likely to have health problems.
● Early death rates are as high in some rural communities as they are in poorer urban neighborhoods.
● African Americans struggle with dramatically higher rates of heart disease than other groups.
● Residents with the lowest incomes had three times the feelings of helplessness versus their higher income peers.
